A fatal incident occurred on the QEW Friday afternoon when a flying tire struck a coach bus, resulting in the death of a Toronto man and leaving three others in serious condition.

According to the officials, the collision, which closed all Toronto-bound lanes for hours, was caused by a wheel coming off a passenger van, striking a pickup truck, and then going through the windshield of the coach bus.
