The UK's MI5 intelligence service has apologised for mistakes that might have contributed to the Manchester Arena bombing in 2017.
22 people and the attacker died at the Ariana Grande concert.
The inquiry concluded on Thursday, after more than two years of hearings and evidence.

Al Jazeera’s Rory Challands reports.
