A new report into the Uvalde, Texas school shooting describes what's being called a "systematic breakdown," highlighting how the safeguards designed to protect students failed.

Newly-released police bodycam footage is also adding to the frustration felt by many, showing the chaos of that horrific day when 19 children and two teachers were killed.
