The US Supreme Court heard Republican objections to the Affordable Care Act, or Obamacare, on Tuesday.
President Donald Trump has long pledged to scrap his predecessor's signature achievement.
But some conservative judges have already signalled they are unlikely to strike down the law.
President-elect Joe Biden has called the Republican case "cruel and needlessly divisive" as the coronavirus pandemic rages in the United States.
