All maternity services in England have been told to take immediate action to improve patient safety.

A review carried out for the Department for Health by Donna Ockenden set out seven areas of care which needed essential action.

Ms Ockenden - a midwifery expert - was asked to look into the deaths of babies and young children at the Shrewsbury and Telford NHS Trust.
