An inquiry into the UK's biggest maternity scandal has found that poor care over nearly 20 years led to the preventable deaths of at least 201 babies and nine mothers.

Babies were stillborn, died shortly after birth, left severely brain damaged or with broken bones due to catastrophic failings, according to the report into Shrewsbury and Telford Hospital NHS Trust.
