An investigation by Newsnight has uncovered evidence of tragic failings in the North East of England Ambulance Service.

Whistleblowers told Newsnight that multiple deaths were not investigated properly, because information was not always passed to coroners and families.

Now, a damning report by the Care Quality Commission has found a deterioration in the service, and downgraded it from ‘Good’, to ‘Needs improvement’. It has also deemed the Trusts' Urgent Care inadequate.

In one example they stated that ambulance crews in the area frequently responded to emergencies without access to life saving drugs.

Newsnight’s UK Editor Sima Kotecha reports.

Warning: This film contains references to suicide and the audio of a 999 call that viewers may find distressing.
