A review into the deaths of two children, Star Hobson and Arthur Labinjo-Hughes, has called for dedicated child protection teams.

The report from the Child Safeguarding Practice Review Panel says the system is too ambiguous and inconsistent - and doesn't serve children or their families and must "change fundamentally".

The national review found that the fatal abuses suffered by Arthur, six, and Star, 16 months, "are not isolated incidents", but reflective of wider problems with poor information sharing and weak decision-making.

This report from Sky News' Home Editor Jason Farrell contains upsetting details of the Star's death.
