Baby Aisha Cleary died in a prison cell after her then teenage mother, Rianna, was left to give birth alone in 2019.

Earlier this year, a coroner ruled that multiple systemic failures led to her death.

But the coroner has decided not to order a prevention of future deaths report - saying he's satisfied with changes made within the prison system.

Rianna disagrees with that decision and has spoken for the first time, exclusively to this programme, about her years-long fight for justice.
